We are a local, independent, non-profit Adult Day Health Care (ADHC) center, licensed and regulated by the Virginia Department of Social Services. Conveniently located across from the old Dublin Middle School, we welcome adults of ALL ages. At Pulaski Adult Day Service & Fall Prevent Center, we believe that everyone deserves meaningful and enjoyable interactions, regardless of their cognitive or physical limitations. Our program is designed to engage participants mentally, physically, and socially. The results are clear: improved sleep, heightened engagement, and a more positive mood for both our participants and their caregivers.
We are proud to employ Registered Nurses, a Board Certified Music Therapist, Certified Nursing Assistants, and Activity Assistants to ensure quality care. Our services include providing a nutritious breakfast, lunch, and afternoon snack. We care for adults aged 18 years and older, offering assistance with eating, toileting, ambulating, and dressing/bathing. In addition, we assess and monitor health conditions, keeping family members informed of any changes to help prevent hospitalization. One spouse expressed their gratitude, saying, “Your service and care make it possible for me to commit to home care for my wife over the long haul.”

Do you want your loved one to be well-cared for and have a better quality of life during the work week? Please consider Pulaski Adult Day Service & Fall Prevention Center. Our program serves people 18 and older who are dealing with being alone during the day, a range of physical issues, or early to mid-stage dementia, and would like to join a program which provides interesting conversation, goofing off, laughing, good food, care, and companionship.
We want to reach out to those families that need our service but don’t know about us or don’t know what adult day healthcare is and how we work with each individual to build on their strengths. Our participants do amazing things here whether it’s rediscovering a talent that they had forgotten or discovering a new interest. We have a Board Certified Music Therapist who taps into memories through music and has a gift for reaching those who have difficulty expressing themselves. We have had people start playing the piano after years of not playing or start painting or singing or working with tools. These activities help open up a whole new area of self-expression and feeling of self-worth and accomplishment.
Our participants love being outside planting, walking and simply enjoying the Therapy Garden. The NRV Master Gardeners come to teach us about plants and lead Garden Club, which taps into memories of growing gardens and a love of growing things. We also play games that build on language and math skills in a fun way. We can be very competitive! When we play ball games, our folks laugh and show good-natured competitive spirit.
We have great discussions on participants' past accomplishments, celebrating their life. We often cook together, measuring, stirring, and reading a recipe. Many of our participants have raised a family through hard times, served our country and helped build this community. So, they deserve our best and it is an honor to know and serve them and their families.

Participants at Pulaski Adult Day Service & Fall Prevention Center entered artwork and flowers and vegetables grown in the Center's gardens into competition at the 2025 New River Valley Fair. Their hard work paid off! Over a dozen ribbons were won, including Best in Show for flowers. Special thanks goes out to the NRV Master Gardeners for their help.

We have begun planting! We have both traditional garden beds and raised garden beds that can be accessed while sitting. We will harvest the product and eat it or enter it in local fairs.
Thanks to the NRV Master Gardeners for their contributions to our program. The Master Gardeners visit us twice a month!
