Community Fall Prevention Services

We believe that everyone has the need for meaningful and enjoyable interactions, despite their cognitive or physical limitations. That’s why our program is focused on engaging our participants mentally, physically, and socially. The results? Better sleep, increased engagement, and an overall better mood — for both our participants and their caregivers.  

 

We care for adults 18 and older with needs such as assistance with eating, toileting, ambulating, and dressing/bathing. We assess and monitor health conditions and alert family members to changes in health, which helps prevent hospitalization.

Community Fall Prevention Services

3 million older adults are treated in emergency departments per year for fall injuries. After a fall, people often become afraid of falling again, even if they weren't injured. They become less active, leading to weakness,  increasing their risk for falling again. Fear of falling or injuries sustained in a fall can rob an individual of their independence.


Our Fall Prevention Services are open to the community, in addition to our adult day service participants.

Caregiver Support

Some of us are the help our loved ones depend on.  Family caregivers need support to do a good job.  Being a caregiver is hard work.  It can be emotionally draining, cause you to miss work, trigger physical ailments, create financial strain, and limit your time with other family members, friends, or your hobbies. Caring for and making decisions regarding older loved ones can even tear families apart.


We provide resources for caregivers of adults, including a support group, Supportive Circle.

Adult Day Services

We provide a safe and secure day program for adults 18 and older that will provide peace of mind to family members of regular participants, health oversight of frail older adults, and social, physical, and cognitive enhancement of function and quality of life. 


We believe that everyone has the need for meaningful and enjoyable interactions, despite their cognitive or physical limitations. That’s why our program is focused on engaging our participants mentally, physically, and socially. The results? Better sleep, increased engagement, and an overall better mood — for both our participants and their caregivers.


We provide: 

  • Assessment of health issues by a Registered Nurse (RN)
  • Medication management as ordered by physician  
  • First aid and CPR as needed 
  • Individualized care plans and quarterly care plan meetings with participants, staff, and family / caregivers 
  • Monthly progress notes; Reminders of annual medical reports.
  • Coded, secure doors to prevent elopement (leaving the activity room unsupervised)
  • Weight monitoring; wheelchair accessible scale
  • Blood pressure monitoring
  • Glucose (sugar) monitoring
  • Bathing services*
  • Assistance with activities of daily living, as needed and according to participant preference:  Feeding, toileting, transferring, ambulation (walking), and anticipation of needs.
  • Morning & afternoon snacks
  • Hot, diet specific, lunch 
  • Two physical activities per day, for example: walking club, horticulture therapy, individual exercise, Tai Chi, A Matter of Balance, Music Therapy, dancing, painting, wood working.
  • Two cognitive activities per day, for example: Music Therapy, reading newspaper, word search, word puzzles, jigsaws puzzles, sing-a-longs, bingo, trivia, brain teasers.
  • Two social activities per day, for example: volunteer-lead programs, dancing, outings, small group activities, one on one interaction between participant and staff / volunteers, men's group activities, women's group activities. 
  • Individual activities, for example: knitting, sewing, reading, computers, word games, drawing.


  • *Shower; or bath in Parker® medical bathtub. The Parker® medical bathtub is designed to be accessible and safe for older adults with impaired mobility. Bath sessions can improve the quality of older adults’ lives with benefits such as relaxation, improved circulation, pain relief and enhanced mobility. The Parker® tub is accessible to older adults who are wheelchair bound with little ability to support their body weight. The built-in, integrated, disinfection and air spa systems ensure the tub’s surfaces are safe, preventing cross infection and meeting all OSHA standards. A hand control can raise or lower the tub for easy access or tilt it for comfort; only one CNA is needed to safety operate the tub.

Licensed for 30 participants per day.

Fall Prevention

A Matter of Balance


Many older adults experience a fear of falling. People who develop this fear often limit their activities, which can result in physical weakness, making the risk of falling even greater.


A Matter of Balance is an evidence-based group program for older adults that is designed to reduce the fear of falling and increase activity levels. The program is a comprehensive approach to maximizing activity engagement and function, and reducing fall risks. It is designed for small groups of older adults living independently, in community settings, or senior housing.

Fall Prevention Center

The Fall Prevention Center will return in the future!  Our vision is a Center that is open to the community, provides comprehensive fall risk assessments, bone densitometer scans, medication review, and intervention planning on an individual basis. Contact us to learn more.
